PAYMENT

Payment is expected at the time of the treatment, which is before the extent of success is determined. This procedure is an elective cosmetic procedure and not reimbursable by government or private healthcare insurers.

 

CONSENT TO TREATMENT

I am undergoing treatment at Freya Aesthetics and Beauty LLC of my own free will. I agree that this procedure is being performed for cosmetic reasons and that no guarantee can be made as to the exact results of this procedure. I understand that reasonable precautions will be taken to prevent complications and that complications from this procedure are rare, but they can and sometimes do occur. 

 

All of my questions have been answered to my satisfaction and I consent to the terms of this agreement. I agree to adhere to all safety precautions and instructions after the treatment. I understand that medicine is not an exact science and acknowledge that no guarantee has been given or implied by anyone as to the results that may be obtained by this treatment.

 

I understand this procedure is “elective” and not covered by insurance and that payment is my responsibility. Any expenses which may be incurred by me in connection with this treatment will be my sole financial responsibility.

Payment in full for all treatments is required at the time of service and is non‐refundable.

 

I represent and warrant to Freya Aesthetics and Beauty, LLC that I am not pregnant, that I a competent adult of at least 18 years of age and that I have full right, power, and authority to enter into this agreement and grant the rights granted hereunder.

 

I understand that if I have questions or concerns regarding my treatment, I will notify Freya Aesthetics and Beauty, LLC in a timely manner so follow‐up and intervention can be provided.

 

I hereby give my voluntary consent for the treatment. I acknowledge that I have fully read and understand the contents of this form and have had an opportunity to ask questions that have been adequately answered. I understand the benefits, risks, and alternatives of the treatment.
